Accountability

Paulina Hupka, CEO and founder of SafeMinds Consulting and Coaching, is appointed to be responsible for personal information under its control.


Identifying Purpose

SafeMinds Consulting and Coaching is collecting personal information and personal health information to provide personalized nutritional, lifestyle and data security recommendations, and to transact purchases of digital products and services.

All information not related to the services provided is excluded from our collection if they do not provide any value to your personalized health recommendations.


Consent

Our agreements and contracts outline our scope of services, limitations and other details about our relationship for our collaboration. Your consent will be required on that form.


Limiting Collection

We collect information only about your wellness, online privacy tools, and health-related details regarding your goals. Anything that does not require being collected will not be requested.


Limiting Use, Disclosure, Retention

We keep your personal information, personal health information, and information about your digital privacy only for the purpose of its collection. If you clearly identify that our collaboration has come to an end, we will delete/shred/disregard the collection of information. Once you clearly informed us about the end of our partnership, we will ask you to sign an agreement stating that you requested to have your Personal Information and Personal Health Information deleted/shredded/disregarded. This form states that we will keep your file active for another 6 months and will complete deletion/shredding/disregarding your information after that period.


Accuracy

To ensure that your information stays accurate and up-to-date, we will perform an audit where we will contact you via email to confirm the accuracy of your information.


Safeguards

All gathered Personal Information and Personal Health Information will be held safely on an external drive.

Additionally, the 3-2-1 rule applies to all our collected data.

All digital tools will require a 3-month password change, 2-factor authentication and regurlarly updated firewall. 

In addition, the external drive and all paper-based forms (intake form, surveys, contracts, agreements, etc.) are locked in a safe location.

Detailed information about our policies and practices relating to the management of personal information will be outlined here, on our website, and in the our Terms and Conditions


Individual Access

Upon request, a client will be informed of the existence, use and disclosure of their personal information and will be given access to that information.


Challenging Compliance

A client has the right to challenge our compliance with PIPEDA. Please contact our appointed employee at info (at) safeminds.ca.
